Hello Dr. Karam,
Just wanted you to know that I appreciate reply and information. It was very helpful! Thank You & God Bless, Tina
Hi,
Sorry for the late reply. Without seeing your eyes in a close up photo it is hard to to tell what the optimal treatment would be. But from what you are describing, it sounds like you have excess skin in the lower lid region. In those cases, I like to do a lower eyelid skin pinch. It is a simple procedure. Thanks.
Dr. Karam

I’m desperately seeking some help for the deep lines or wrinkles that appear under eyes when I’m smiling. I don’t have the typical crow’s feet, just these deep lines under my eyes that appear when I smile. They are slightly visible when not smiling, nothing significant. Are there any treatments to address this problem, because I certainly don’t want to stop smiling.
Thank you for your time and much needed help and guidance.
Hello and Happy New Year, Dr. Karam,
I’m a highly sensitive person that has many allergic reactions that mostly appear on my face. Do you have any suggestions for the reduction of excess facial skin under the eyes? Thank You, Tina
Sorry for the late reply. But here are my thoughts. The decision of how deep of a peel is required is dependent on several factors. 1) It depends on the severity of the age-related and sun-induced changes. The more significant, the deeper the peel that is required. 2) How much downtime can the person tolerate. If very little then obviously a 3 TCA etc is more than you can handle. The results are generally better the for the deeper peels.. ie 25 TCA and above. However, if you can’t take the time off, I usually do a series of lighter peels (15% TCA) which will, after 3-4 spaced 6 or so weeks apart, give you results that are approaching that of the deeper peels. These are still considered MD-level peels. The lower the concentration the shorter the potential downtime.
In general, I am a big believer of chemical peels.. the results can be really good and often times much less expensive than other types of skin resurfacing.
I hope this helps.

Hello Dr. Karam,
I have been considering getting a chemical peel done. The dermatologist I spoke with said there are two types to consider. One type is a peel done by the anestitician (spelling wrong I know) which is much cheaper (90.00). The other must be done by him (Jessner and TCA peel) and cost 450.00. I have some sun damage spots and discoloration, fine lines, few scars, and large pores. He said I would be very red x 1wk and I would have to be off work for that time due to my employment (RN in critical care). Total healing time is 3 wks. What are your thougths? Thank you for your time!
